RESUMEN En este trabajo se realizó una revisión narrativa de los principales hallazgos en la literatura médica sobre las manifestaciones clínicas de la encefalitis autoinmune pediátrica (EAP) no mediada por anticuerpos anti receptor del ácido N-metil-D-aspartato (NMDAR). Las EAP que se destacaron se asocian con anticuerpos específicos como el complejo de canales de potasio dependiente de voltaje, la decarboxilasa del ácido glutámico, el receptor del ácido alfa-amino-3-hidroxi-5-metil-4-isoxazolpropiónico y el receptor ácido-gamma-aminobutírico. El diagnóstico de esta patología es un desafío en pediatría debido a la complejidad de las manifestaciones psiquiátricas y neurológicas generadas por la afectación de la corteza, el sistema límbico, el tallo cerebral, los ganglios basales y el cerebelo. A su vez, la comprensión de sus síntomas permite identificar la superposición en las presentaciones clínicas entre los diferentes tipos de EAP y el diagnóstico diferencial con otras enfermedades inflamatorias del cerebro, infecciones, enfermedades metabólicas y trastornos psiquiátricos. El conocimiento biomédico y la sospecha clínica de las EAP no NMDAR establece un campo de investigación que crece en neuropsiquiatría y favorece el diagnóstico y tratamiento de las encefalitis subdiagnosticadas.ABSTRACT This work presents a narrative review of the main findings in the medical literature on the clinical manifestations of pediatric autoimmune encephalitis (PAE) not mediated by anti-N-methyl-Daspartate acid receptor (NMDAR) antibodies. Prominent PAEs are associated with specific antibodies, such as the voltage-gated potassium channel complex, glutamic acid decarboxylase, alpha-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor, and alpha-amino -3-hydroxy-5. -methyl-4-isoxazolepropionic acid receptor. -gamma-aminobutyric. Diagnosis of this pathology is a challenge in pediatrics due to the complexity of psychiatric and neurological manifestations generated by involvement of cortex, limbic system, brainstem, basal ganglia and cerebellum. In turn, understanding its symptoms allows identifying the overlap in clinical presentations between the different types of PAE and the differential diagnosis with other inflammatory diseases of the brain, infections, metabolic diseases and psychiatric disorders. Biomedical knowledge and clinical suspicion of non-NMDAR PAE establishes a growing field of research in neuropsychiatry and favors the diagnosis and treatment of underdiagnosed encephalitides.
- Muertes por suicidio en Personas Mayores y género masculino en Chile: Una reflexión pendienteel abril 20, 2024 a las 7:52 am
RESUMEN Introducción: Los avances tecnológicos y científicos del último siglo, entre otras cosas, han permitido aumentar la esperanza de vida al enfrentar exitosamente la mortalidad por causas infecciosas. No obstante, hoy se enfrentan nuevos desafíos en torno a la mortalidad, como las crecientes tasas de suicidios globales que afectan con más énfasis a hombres y personas mayores. Desarrollo: La mayor incidencia de muertes por suicidio en hombres mayores, profundiza la necesidad de analizar aquellos factores que puedan estar relacionados, para luego articular respuestas oportunas en la prevención de este. La vivencia de género y de vejez asoman como factores a revisar para la comprensión del fenómeno. En este caso, el entendimiento de la vivencia de masculinidad hegemónica dominante podría aproximarnos a la relación emergente entre género masculino, vejez y suicidio. Conclusión: Los constructos sociales de vejez y masculinidad podrían aportar relevantes antecedentes para aproximarse al fenómeno global que relaciona a los hombres mayores con las muertes por suicidio, factores necesarios de revisar para comprender cómo estos imaginarios sociales pueden influir en sujetos a lo largo del curso de vida. El análisis con perspectiva de género podría facilitar la prevención y respuesta oportuna frente al fenómeno, favoreciendo la salud mental de las personas diseñando e implementando planes y programas integrales de salud pública que consideren las variables como sexo, género, factores sociales y biológicos, con una perspectiva de curso de vida, abordando sus condicionantes desde etapas tempranas y disminuyendo así el impacto durante la vejez.ABSTRACT Introduction: The technological and scientific advances of the last century, among other things, have increased life expectancy by successfully coping with mortality from infectious causes. However, today new challenges are being faced around mortality, such as the growing global suicide rates, which affect men and the elderly more emphatically. Development: The higher incidence of deaths by suicide in older men deepens the need to analyze those factors that may be related, in order to articulate timely responses in its prevention. The experience of gender and old age appear as factors to review for the understanding of the phenomenon. In this case, the understanding of the experience of dominant hegemonic masculinity could bring us closer to the emerging relationship between the masculine gender, old age, and suicide. Conclusion: The social constructs of old age and masculinity could provide relevant background to approach the global phenomenon that relates older men to suicide deaths, factors that need to be reviewed to understand how these social imaginaries can influence subjects throughout lifetime. Analysis with a gender perspective could facilitate prevention and timely response to the phenomenon, favoring people's mental health by designing and implementing comprehensive public health plans and programs that consider variables such as sex, gender, social and biological factors, with a life course perspective, addressing its conditions from early stages and thus reducing the impact during old age.

RESUMEN Introducción: Existe una creciente evidencia investigativa sobre la relación entre el síndrome ortoréxico y el perfeccionismo. Objetivo: Se efectúa un análisis descriptivo interpretativo e integrador acerca de la relación entre el perfeccionismo con la ortorexia nerviosa, destacando los aspectos clínicos, etiopatogénicos y nosológicos. Método: Se realizó una búsqueda bibliográfica sobre la relación entre el síndrome ortoréxico y el perfeccionismo mediante las bases de datos Medline/PubMed, SciELO y textos especializados. Resultados: El término ortorexia se ha concebido como una fijación patológica hacia una alimentación equilibrada y saludable (Bratman, 1997), implicando restricciones dietéticas drásticas, con interferencia significativa en las relaciones sociales e insatisfacciones afectivas, que, además favorecen la conducta alimentaria desadaptativa. Etiopatogénicamente, existe controversia respecto a si es un mero estilo de vida, o puede incluirse definitivamente como un trastorno en los espectros alimentario u obsesivo-compulsivo. De hecho, posee similitudes, diferencias e incluso traslapes con ambas entidades psicopatológicas. Se destaca como rasgo relevante común, el perfeccionismo, orientado hacia sí mismo, hacía los demás, y socialmente prescrito, que favorece la tendencia a desarrollar conductas ortoréxicas elevadas. Conclusiones: El perfeccionismo surge como una característica sobresaliente, positivamente correlacionada con el síndrome ortoréxico, cuya presencia se superpone con los ámbitos, tanto alimentario (especialmente la anorexia nerviosa) como obsesivo-compulsivo.ABSTRACT Background: There is a growing research evidence on the relationship between orthorexic syndrome and perfectionism. Objective: An interpretive and integrative descriptive analysis on the relationship between perfectionism and orthorexia nervosa, highlighting the clinical, etiopathogenic and nosological aspects is carried through. Method: A bibliographic search on the relationship between orthorexic syndrome and perfectionism using the Medline/PubMed, SciELO and specialized text databases was carried out. Results: The term orthorexia has been conceived as a pathological fixation towards a balanced and healthy diet (Bratman, 1997), implying drastic dietary restrictions, with significant interference in social relationships and affective dissatisfactions, which also favor maladaptive eating behavior. From an etiopathogenic perspective, it is controversial if orthorexia nervosa is a mere lifestyle, or definitively must be included as a disorder in the eating or obsessive-compulsive spectrum. In fact, the orthorexic syndrome has similarities, differences and even overlaps with both psychopathological entities. Perfectionism stands out as a common relevant trait, oriented towards oneself, towards others, and socially prescribed, which favors a tendency to develop high orthorectic behaviors. Conclusions: Perfectionism emerges as an outstanding characteristic, positively correlated with the orthorexic syndrome, overlapping with both eating (especially anorexia nervosa) and obsessive-compulsive areas.
- Efectos psiquiátricos de la hipoxia normobárica intermitente. Un Estudio prospectivoel abril 20, 2024 a las 7:52 am
RESUMEN Introducción: La hipoxia intermitente consiste en la administración de aire reducido en oxígeno por episodios, para lograr hipoxia en sangre, alternados con intervalos de normoxia. Tiene uso terapéutico en varias patologías médicas, pero sus efectos psiquiátricos no han sido estudiados. Se han descrito efectos neurogénicos y en modelos animales puede ayudar a prevenir depresión y ansiedad en respuesta al estrés. Objetivo: Determinar la existencia de efectos psiquiátricos de la hipoxia normobárica intermitente en seres humanos. Método: Estudio prospectivo descriptivo. Se incluyeron pacientes mayores de 18 años, que tuvieran programado un plan de hipoxia normobárica intermitente en la clínica Hipoxykine. Se utilizó una ficha de registro y la escala breve de síntomas (BSI) que fue realizada al inicio, mitad (segunda semana) y finalizado el tratamiento (cuarta semana). Resultados: Se incluyeron 22 participantes (50% mujeres) con una mediana de edad de 46 años. Hubo una reducción del índice global de gravedad del BSI estadísticamente significativo (p <0.05) al comparar tanto la primera como la segunda medición respecto a la final. Respecto al análisis según género, esta diferencia fue estadísticamente significativa (p <0.05) en hombres. Conclusiones: La hipoxia normobárica intermitente no produce ni empeora síntomas psiquiátricos y podría tener potencial terapéutico. Se requieren nuevos estudios prospectivos.ABSTRACT Introduction: Intermittent normobaric hypoxia consists of the administration of air reduced in oxygen by episodes, to achieve hypoxia in blood alternated with intervals of normoxia. It has a role in brain development and neuroprotection. However, in the literature there are only studies in animal models. Objective: To determine the existence of psychiatric effects of intermittent normobaric hypoxia in humans. Method: Prospective descriptive study. We included patients over 18 years of age who had an intermittent normobaric hypoxia plan scheduled at the Hipoxykine clinic. A record sheet and the brief symptom inventory (BSI) were carried out at the beginning, middle (second week) and end of treatment (fourth week). Results: We included 22 participants (50% women) with a median age of 46 years. There was a reduction in the overall severity index of the statistically significant (p <0.05) when comparing both the first and second measurements with respect to the final. Regarding the analysis according to gender, this difference was statistically significant (p <0.05) in men. Conclusions: Intermittent normobaric hypoxia does not produce or worsen psychiatric symptoms. It could have therapeutic potential. Further prospective studies are required.
